If your insurance is not on this list, please contact your insurance or our office to determine if we are in network. If we are not in network, we will still accept most insurances on an out-of-network basis. We do not accept Idaho Medicaid.

Copay/Coinsurance is due at the time of service. We will verify your benefits prior to your first appointment to assure that the appropriate amounts are collected. We also encourage you to call your insurance's customer service number on your card if you have questions about coverage/copay/coinsurance/deductibles. We accept checks and most major credit cards. If you wish to pay by cash, we ask that payments are exact as we do not have the ability to make change.
